What are some common causes of pelvic pain and discomfort?
Your pelvis is the crossroads of your body—where your spine, hips, and core all meet. When things get out of balance, your whole body feels it.
The anatomy of your pelvic region includes:
- Pelvic bones (ilium, ischium, pubis, sacrum, coccyx)
- Hip joints
- Muscles of the pelvic floor, glutes, and low back
- Ligaments connecting the pelvis to the spine and hips
- Nerves, including the pudendal and sciatic nerves
With the complex anatomical systems that run through and attach to your pelvis, your pain could be from a number of conditions.

Pelvic Floor Dysfunction
- Feels like: Pressure, aching, or heaviness in the pelvis
- What it is: Poor coordination or weakness of the pelvic floor muscles
- Often triggered by: Childbirth, chronic constipation, or trauma
SI Joint Dysfunction
- Feels like: Sharp, stabbing pain near the tailbone or deep in one hip
- What it is: Misalignment or irritation of the sacroiliac joints, which connect your pelvis to your spine
- Often triggered by: Prolonged sitting, pregnancy, or uneven standing postures
Groin Strains and Tendonitis
- Feels like: Sharp groin or upper thigh pain, especially with movement
- What it is: Overuse or sudden injury to the muscles and tendons of the inner thigh
- Often triggered by: Sports injuries, overuse, or sudden twisting movements
Pubic Symphysis Dysfunction
- Feels like: Sharp or burning pain right at the pubic bone, especially during pregnancy or postpartum
- What it is: Instability or misalignment of the joint where the two halves of the pelvis meet in front
- Often triggered by: Pregnancy, especially in the third trimester, or postpartum recovery
Menstrual and Uterine Pain
- Feels like: Intense cramping, low back pain, and pelvic discomfort during your cycle
- What it is: Tight pelvic muscles and inflammation that amplify period pain
- Often triggered by: Hormonal changes, pelvic floor tension, or endometriosis
Maignes Syndrome
- Feels like: Pain radiating from the mid back, wrapping around the waist, and traveling toward the groin—often with accompanying low back, hip, and groin discomfort
- What it is: Irritation of the nerves that leave the mid back, caused by misalignment and resulting in radiating pain that can mimic groin issues
- Often triggered by: Lack of mid back mobility, wearing really tight clothing, or being overweight
What is chiropractic treatment for pelvic pain like?
We generally use a combination of treatments, including gentle adjustments, joint mobilizations, soft tissue therapy, and movement coaching.
Your treatment could include:
- Activator adjustments for pubic symphysis and SI joint misalignments
- Drop table for safe and effective pelvic adjustments
- Myofascial release for tight pelvic floor and hip muscles
- Gentle pelvic floor muscle engagement or relaxation coaching

Our diagnosis process includes:
- Detailed history of your symptoms, childbirth, surgeries, and activities
- Orthopedic and neurological tests
- Palpation of muscles, joints, and ligaments
- Movement and posture analysis
- Referral for pelvic floor PT, imaging, or OB-GYN care when appropriate
How long does it take to feel better?
Everyone’s healing journey is different, but here’s what we typically see:
- Acute groin strains or SI joint misalignments often feel better in a few visits
- Pelvic floor dysfunction or postnatal pelvic pain may take a longer course of care. These conditions often require regular check-ins and home exercises
- Menstrual discomfort and related tension can often be eased with monthly care and lifestyle coaching
